By submitting this nomination, as the nominee, I understand that this nomination is in no way a guarantee of winnings for myself or my nominee, rather an opportunity to be evaluated for the chance to be the final recipient of the GreatLIFE Gives Fund award. I give permission for a member of the GreatLIFE Gives committee to contact me for further information should they see fit. Should my nominee be selected as a finalist, I understand that we (myself and/or nominee) may be required to participate in a finalist interview with a member(s) of the GreatLIFE Gives committee. Should my nominee be the recipient of the GreatLIFE Gives Fund, I understand that we will be required to attend an award ceremony that will entail interviews and video recordings of the event which will be used in future marketing materials and/or publications.
